Effective immediately MOD Mayor's Office on Disability will no longer accept Public Records Request (PRR) on this portal. On April 1, 2025, Mayor's Office on Disability has been renamed to SF Office on Disability within the Human Services Agency (HSA). Please visit https://www.sfhsa.org/about/policies-procedures/public-records-subpoenas on how to file PRR with HSA.
