URGENT:  Please NOTE that certain provisions of the San Francisco Sunshine Ordinance have been temporarily suspended under recent Supplements to the Proclamation Declaring a Local Emergency, including the Immediate Disclosure Provision and the 10 day period in which to provide or withhold documents.  Compliance with the California Public Records Act and other provisions of the Sunshine Ordinance remains in effect.  See the orders under Mayoral Proclamation in DOCUMENTS tab. 

 

REQUESTS FOR NON-PARTICIPATING DEPARTMENTS

If you have a request for a department not on that list, visit that department’s website and follow the instructions there for submitting a request. Requests submitted through this portal for non-participating departments will be closed out.

REQUESTS FOR MULTIPLE DEPARTMENTS

If you would like to submit a request to multiple departments, please submit a separate request to each department.

INCLUDING DOCUMENTS WITH REQUESTS

If you would like to include a document with your request you can upload it to the request after the request has been submitted. 

ANONYMOUS REQUESTS

Requesters who wish to remain anonymous do not need to provide a name, but we ask that you provide a phone number, email address, or mailing address of your choosing so that we may contact you when your records are ready, or if we have questions about your request or need to seek clarification from you regarding your request. A requester who wishes to preserve anonymity may choose to create an email address without any identifying information in it.

Any personal contact information (other than name) that a requester provides will be kept confidential by the City and will not be publicly disclosed. If you choose not to provide any contact information on this form, the only way the City will be able to contact you regarding your request will be by posting a public message on this portal requesting that you contact us; therefore, you will need to check this site regularly for any such postings.
